About Us

Why this project matters

We're working with communities across rural and remote Australia to explore new, easier ways for people to check their heart health.

Right now, getting a heart health check can involve multiple appointments, travel, and time, especially for people living outside major cities. This project is looking at different options, such as at-home checks or pharmacy-based checks, to make the process simpler and more accessible.

People who take part can try one of these new approaches and will receive a heart health assessment and guidance on what to do next, including when to see their GP.

This project is co-designed with rural communities, meaning local people, healthcare providers, and researchers working together.

By taking part, you're helping to develop a system that could make heart health checks easier, more accessible, and better suited to rural and remote communities across Australia.

Take part in our community project

We're inviting people to be part of a community project exploring new, easier ways to check heart health.

These new ways include options to check your heart health at home, through a local pharmacy, or at a local community health centre, making it simpler and more convenient to stay on top of your heart health.

By taking part, you're not just trying something new, you're helping shape how heart health checks could work better for people in communities like yours.

Your involvement will help create a system that is easier to access, more flexible, and better suited to everyday life.

Where is this happening

This stage of the project is taking place in communities across central and north-west Victoria and south-west New South Wales: Mildura, Merbein, Buronga, Gol Gol, Wentworth, Dareton, Lockington and Ouyen.

Who can take part?

You may be able to take part if you:

  • are aged 45 to 79 years
  • have not had a Heart Health Check with your GP in the past 12 months
  • have not been diagnosed with heart disease, chronic kidney disease, or inherited high cholesterol

It's a small step that could make a big difference.

Reduce your Risk of Heart Disease

Risk Factor Reduction Program

After participating in a heart health check, you will receive an assessment of your heart health along with an invitation to access our risk factor reduction program, which is designed to help you reduce your risk of heart disease if needed.
